SEAT BELT FAILING TO RESTRAIN PASSENGER

​Problem - My 2004 Gallardo has always had a problem with the passenger seat belt not restraining the occupant. 

Possibility 1 - I have never heard of this but get it replaced immediately for safety purposes.
